Registration Form
You can register online by filling out the form on the Walkathon page, or you can download the form below and do either the following:
- Download, print and fill out the Walkathon_Registration. Afterward, you can drop it off at our office, or you can mail it to us; or
- Download and fill out online. Save the document and then email it to info@epilepsyniagara.org;
- Fill out the online registration form below.
Pledge Form
You can use this form to collect pledges, and you can download and print as many as you need. Any cheques collected must be made payable to Epilepsy Niagara. The team that raises the most funds will win a prize!
